JOB DESCRIPTION
  1. Job responsibilities include those listed in competency document.
  2. Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.
  3. Provides administrative support to Care Management teams as needed to support workflow process and outcomes to include: answer multiple and designated telephone lines, prioritize and route calls, demonstrates effective and interpersonal communication skills, and utilizes computer skills at an intermediate level.
  4. Assist Care Manager in downloading meters, pumps, or sensors.
  5. Understands and recognizes appropriate storage and safety of medications (such as alcohol, saline, insulin or other agents utilized for Diabetes Services).
  6. Maintains competency in, but not limited to, the following care task: vitals signs, weights, blood glucose meters.
  7. Coordinates patient transport when requested.
  8. Assist with front office duties as needed. Enters referral notes, status of bed offers, and other details as necessary.
  9. Independently and proactively initiates the follow-up with care team members to ensure appropriate care coordination.
  10. Documents accurately and thoroughly all transactions regarding care and status that occur during their shift in the medical record according to policy.
  11. Performs all other duties as assigned
REQUIRED SKILLS

Qualifications /Training:

Proficient with Microsoft Word and Excel. Familiarity with glucose monitoring, vital signs. Experience with data entry. 

Licenses/Certifications/Registrations/Education:

Minimum of a High School Diploma or equivalent (GED) from an accredited school.
