Job Summary •    Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values. •    Shall demonstrate leadership and commitment with respect to the quality management system by: •    promoting the use of the process approach and risk-based thinking •    engaging, directing and supporting persons to contribute to the effectiveness of the quality management system requirements •    promoting improvement. •    Serves as an access point resource for physicians, staff and patients.  •    Performs medical record review to assess for appropriate of admissions and works with the Emergency Room and attending physicians to divert unnecessary admissions.  •    Communicate with physicians and nurses regarding non-emergent admissions based on bed availability, treatment plan and admission criteria; initiate and maintain communication throughout the day  •    Coordinate urgent/emergent admission authorization; coordinate out of network admission, lack of referral from primary care provider, no insurance, and other special admissions with attending physicians and others.  •    Collect, investigate and disseminate clinical data to other Patient Flow Center personnel regarding diagnosis and treatment plans and insurance data for evaluation of admission criteria •    Responsible for Leave of Absence (LOA) monitoring •    Coordinate with case management on patients with long-term diagnoses, extended stays and other high risk diagnoses to ensure that patients remain on proper clinical pathways •    Attend daily rounds serving as an escalation point to remove barriers to post-acute placement •    Ensure compliance with standards related to pending discharges for current and following day in coordination with social workers and unit case managers •    Monitors and documents avoidable days and recommends changes in services or processes.  •    Monitor Code 44 worklist to ensure completion of workflow in a timely manner  •    Responsible for proactively monitoring acuity scoring throughout hospitals  •    Perform other related duties as assigned

Work Schedule: 80 Hours Biweekly. Full Time. Day Shift

Qualifications /Training: •    3 years minimum experience in the acute care setting •    1 year of case management or utilization management experience  •    Self-direction required for daily work.

Licenses/Certifications/Registrations/Education: •    Associates Degree in Nursing, required.   •    Current license as Registered Professional Nurse
