Clinical Pharmacy Fellow (Coastal Region) – Care Management

Job Summary  Administrative •    Maintains and submits a monthly reconciliation report and productivity report for the supervising provider, chief operating officer, and clinical coordinator •    Data collection for quality assurance measures for assigned clinical services •    Assist in the development of new patient care programs as needed •    Assists in the development and execution of business plans and goals for clinical services •    Set short-term and long-term goals for clinical services in the best interest of the patients organization, providers, other healthcare professionals and staff of collaborating parties •    Develop systems to measure quality assurance for assigned programs •    Provides feedback regarding quality measures and clinical service agreements to help improve data collection and quality improvement on an ongoing basis •    Job responsibilities include those listed in competencies document Educational •    Provides in-service presentations for the education and training of internal team members, external team members and community partners, as needed

Clinic •    Provides direct patient care services under the direction of their assigned medical •    provider(s) •    Reviews and updates the collaborative practice agreements between medical provider and pharmacist team as needed  •    Builds and maintains a professional relationship with all team members of the •    medical practice and members of the community •    Follows appropriate protocols and scope aligned with clinical service agreements •    and clinical service protocols •    Provide and obtain feedback from internal and external team members of the •    pharmacy and collaborating clinic •    Actively participates in direct patient care programs, medication therapy •    management services, medication use systems, and student pharmacist •    experiential training  •    Knowledgeable in medication therapy management and chronic disease state management (i.e. diabetes, heart failure, COPD, hypertension, tobacco cessation, etc.) •    Exhibits clinical and professional skills to positively interact with patients & all members of the healthcare team 

Other •    Contributes to the continued development of the practice of the profession of pharmacy. •    Participates in organizational committees related to medication therapy management.

Work Schedule: 80 Hours biweekly. Full Time. This position will begin in July 2022 and will be physically located at the coast (Loris, Little River or Myrtle Beach area) in a McLeod Physician Office. 

Qualifications/Education/ Licenses: •    Baccalaureate degree or PharmD. Degree from an accredited school of pharmacy, required •    American Society of Health-Systems Pharmacy (ASHP) accredited PGY1, preferred •    Ability to work independently •    Ability to independently read and interpret evidence based medicine guidelines •    Completion of ASHP accredited PGY1 pharmacy practice residency or at least 2 years of MTM, Population health management, ambulatory care experience •    Current, unrestricted license to practice pharmacy in the state of South Carolina, required •    Immunization certificate
