Community Navigator – Access To Health
Essential Functions

- Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.
- The Community Care Navigator will provide assistance with coordination of access to health care services and provider referrals to low income uninsured or underinsured clients eligible for the Access Health/HOP program.
- Will help to facilitate access to health care by reducing or resolving barriers for assigned and/or referred clients.
- Responsibilities of the Community Care Navigator include, but are not limited to, case coordination, patient education, travel/home visits, and direct follow-up related to accessing and/or retaining health care and supportive services.
- Must use own car for travel to remote sites, often in rural, isolated areas with proof of current auto insurance.
- Ability and willingness to provide emotional support, encouragement, and motivation to patients.
Work Schedule: 80 Hours Biweekly. Full Time.
Qualifications /Training: • Knowledge base of community services and resources • Must be able to handle potential problems related to safety issues for staff and patient in all settings. • Knowledge of medical terminology required. • Basic computer skills (Excel and Word) • Good communication skill, such as listening well, and using language appropriately
Licenses/Certifications/Registrations/Education: • High School Diploma or GED • willing to obtain BLS
