Discharge Planner – Case Management

Essential Functions

1.    Maintains a professional image and exhibits excellent customer relations to patients. visitors. physicians. and co-workers in accordance with our Service Excellence Standards and Core Values. 2.    Under direction of the RN and/or social worker completes referrals for post-acute services for patients and assists with tasks that do not require a clinical license or degree. The position responsivities include the following activities: a)    arrange post-discharge services; b)    actively seeks alternative levels of care as indicated by the assessment: c)    create and follow up on electronic referrals: d)    review patient choice letters with patients/families for required signatures: e)    facilitates system process improvement to remove barriers to discharge: f)    provide follow up Important Message to Medicare patients prior to discharge: g)    communicate with patients, families and other members of the care team: h)    reviews/discusses discharge planning activities at team meetings. i)    complete tasks assigned by Case Manager and Social Work staff; j)    make copies, send faxes and complete phone calls: k)    complete process reviews or audits as requested; and other duties as assigned.

Work Schedule: 80 Hours, Biweekly.

Qualifications/Training: One-three years related experience in a healthcare setting Computer literacy  Excellent communication skills Demonstrated problem solving skills and excellent organizational skills 

Education/Licenses and/or Registrations/Certifications: Required: High School Diploma or GED Required. Associates Degree Preferred
