Nurse Hospice Weekend On Call
1. Plans, organizes, and provides patient care for McLeod Hospice accordingly through demonstration of the following competencies.
- Will share responsible of calls to agency from 5:00 PM Friday until 8:30AM Monday
- Manages calls on weekends to assure patients receive appropriate, quality care.
- Will Share responsibility for admissions, nursing assessments, and prn visits based upon the calls received and patient need.
- Coordinates and manages daily workload of the weekend staff. Ensures appropriate completion of all services with timely completion of documentation that is in compliance with state and federal regulations.
- Assures competent performance of weekend staff in order that quality of care and services are provided.
- Notifies attending physician promptly of pertinent observations and reactions, incidents, or abnormal test results from the weekend; and to obtain physician’s orders as needed.
- Communicates changes in patient needs to other members of the interdisciplinary team; and makes referrals as appropriate.
- Promotes safe, effective patient and office environments, and equipment use.
2. Coordinates patient care to ensure that care meets the needs of the patients and families during weekends and after hours.
a. Complete home visits when required for:
- Death, Suspected Death
- Severe or uncontrolled pain
- Nausea/Vomiting not resolved with present medications
- New onset of seizures or suspected seizures
- IV problems
- Occluded tubes
- Respiratory difficulty or distress
- Report of patient falling
- No BM for four (4) days or more (if taking nourishment)
- No urine for eight (8) to twelve (12) hours (if taking fluids and having discomfort)
- Increased anxiety, agitation, restlessness
- Duplicate calls regarding the same problem (if the patient or family calls more than one time)
- Patient, family, or caregiver perceives a problem and requests a visit
- If the patient or family calls to say they are going to the Emergency Department, the RN will go to the Emergency Department to provide information and assure that the Hospice Plan of Care is reviewed.
b. Completes required documentation and statistics for administrative and regulatory purposes.
c. Demonstrates compliance with operating standards.
d. Reports to Clinical Supervisor/Director and suggestion of recommendation for positive changes with the program.
e. Demonstrates an ability to work effectively and communicate frequently with other members assigned.
f. Assures the delivery of quality services through participation in the quality improvement process.
3. Develops cooperative and supportive relationships within Hospice and other departments to ensure that interprofessional goals and objectives are met accordingly through demonstration of the following competencies.
a. Demonstrates compliance with the vision, mission, values, strategic plan and goals for the department, units, job positions and McLeod Organization.
b. Maintains an open, empathic communication style that focuses on problem solving to the mutual benefit of all parties.
c. Works with other agencies and promotes good community relations.
d. Accepts patient assignments willingly. Demonstrates positive behavior in all aspects of role.
