JOB DESCRIPTION
- Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.
- Functions as a part of the multidisciplinary Neuroservices team for provision of care to the Neurology, Stroke, Neurosurgical and Thrombectomy patients.
- As needed, navigates Neuroservices patients through the trajectory of care by assisting with facilitating patient appointments to care providers.
- As needed, provides education to patient and family members about specific disease processes.
- Develops and provides patients with comprehensive treatment summaries/survivorship plans of care upon completion of treatment.
- Serves as liaison between patients and clinical care providers, facilitating communication and coordination of care as needed.
- Participates in site specific Neuroservices conferences.
- Develops and provides educational needs to physician and staff for Neuroservices.
- Manages data and outcomes for Neuroservices program.
- Promotes the profession of Neuroservices navigation through patient in quality improvement initiatives within the Neuroservices program.
- Maintains working knowledge of current therapies and treatment details for the specific disease for the Neuroservices population served.
- Serves as a clinical expert for Thrombectomy and Stroke care by keeping current with evidenced based practice, literature review and educational offerings.
- Develops and maintains professional relationships with physicians/care providers and other professional colleagues within the Neuroservices division.
- Is knowledgeable of and adheres to DNV Stroke accreditation standards.
- Participates in Neuroservices Committees as requested.
- Participates in community events, such as community health fairs, as requested.
- Represents the organization in a positive and professional manner at all community events.
- Completes annual training and annual educational requirements timely.
- Performs other tasks, duties or responsibilities as assigned or need for the effective, safe and efficient functioning operation of the hospital.
Hours: 8:00am-4:30pm Monday through Friday
- At times additional hours may be necessary to accomplish the job, for meetings, and for other duties as assigned
Licenses/Certifications/Registrations/Education:
- Must possess a valid SC license – RN, BSN
- Advanced Certification as approved by AVP or VP within 1 year of hire
