Palliative Care Specialist (RN) –  McLeod Hospice

Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values. The palliative care specialist is a registered nurse who is responsible for providing case management services to assigned populations.  He/she utilizes a collaborative interdisciplinary approach to anticipate, integrate and coordinate the patient’s plan of care, discharge plan and re-entry into the community.  The goal of his/her intervention is to meet the needs of patients in order to optimize clinical and resource outcomes and to monitor the effectiveness of the plan and its outcomes.  The palliative care specialist serves as a patient/family advocate and acts as a communication link between the physician, patient, family, health care team, and community resources and payers to increase the quality of care for Palliative care patients.  He/she anticipates potential delays in the health care process and acts proactively to avoid these delays. The palliative care specialist ensures that services are provided at the most appropriate level of care and that there is a smooth progression of the patient through the health care system. Demonstrates competency in, but not limited to, the following in the Palliative Care Program (refer to McLeod Health Competency Checklist):

Schedule: Full Time; 80 hours biweekly.

Qualifications /Training:

  • Basic knowledge of maintaining statistical information.
  • Prefer candidate to have 3 – 5 years of work experience in the hospital setting, or related experience. 
  • Team coordination or supervisory experience helpful but not required.
  • Requires strong organizational and interpersonal skills and the ability to think critically, problem solve and to work independently. 
  • Presentation, training and public speaking skills needed.
  • Excellent written and verbal communication skills are required. 


  • Must possess a valid SC license- RN.
