Transfer Coordinator (RN Required) – Patient Flow Center
Job Summary
1. Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values. 2. Shall demonstrate leadership and commitment with respect to the quality management system by: (a) promoting the use of the process approach and risk-based thinking (b) engaging, directing and supporting persons to contribute to the effectiveness of the quality management system requirements (c) promoting improvement. 3. Facilitates transferring patients from internal and external facilities into the McLeod Health System. Works collaboratively to optimize patient placement in an effort to maximize enterprise capacity. 4. Serves as the initial point of contact from external and internal providers and obtains the necessary information to initiate patient transfers into the McLeod Health System. 5. Ensures accurate documentation of future admission demand for the hospital while working with flow team to ensure acceptance of the right patients at the right time. 6. Receives transfer request from physicians, mid-level providers or representatives from internal and external facilities. Information gathered includes demographic, insurance and clinical data. 7. Coordinates referrals to appropriate medical service as needed for referred patient. 8. Connects referring parties with accepting McLeod provider to determine appropriateness of transfer. 9. Acts as a liaison between referring hospitals and physicians on issues pertaining to placing patients. 10. Ensures patients receives proper LOC when they arrive. 11. Assist with bed placement as needed. 12. Ensure patients can safely move in and out of hospital by coordinating clinical hand off and dispatch service. 13. Performs follow up communication with financial counselors, physicians, case managers and nurses to help expedite transfers to the McLeod Health system facility. 14. Demonstrate knowledge of hospital, departmental, and health systems policies and procedures. 15. Actively participates in the maintenance of 24/7 daily systems to maximize patient flow and optimize bed capacity across the enterprise. 16. Provides feedback to the supervisors related to process delays. 17. Meets regularly with team members to discuss, problem-solve, and develop mitigation strategies related to improvement of patient flow. 18. Actively engages in daily implementation of strategic goals as they relate to capacity management, patient flow and department development. 19. Develops and promotes long term relationships with referring facilities.
Work Schedule: 72 Hours. Full Time. 11am – 11pm
Qualifications /Training: • Minimum 3 years of acute care nursing • Advanced knowledge of Windows based computer systems • Written and verbal communication skills with persons of all ages and diverse backgrounds • Knowledge of EMTALA and other compliance regulations
Licenses/Certifications/Registrations/Education: • Associate degree Nursing • Current license as Registered Nurse required
