JOB DESCRIPTION

Assesses family or significant others to determine specific needs for care plan.

Within first two weeks after patients death, a letter of available bereavement services.

Initiates the bereavement assessment and is documented in the records within 8 weeks after patients death.  This assessment may be made by phone or a face to face contact.

Develops a bereavement care plan that reflects services to be provided, frequency and duration of delivery and primary persons furnishing those services.

Demonstrates skill in identifying survivors high risk factors in determining the scope and type of bereavement counseling, interventions, and/or referrals.

Progress notes will focus on dates of contact, type of contact/efforts to contact, bereavement response and progress, particular needs or concerns as expressed by bereaved or noted by bereavement/continuing care staff or hospice team, any changes in plan, any interventions or referrals.

Provides counseling and support services to families. 

Provides individual, family and group counseling. 

Assists Coordinator in providing bereavement training and support for volunteers and other Hospice staff

The Counselor will assist Coordinator with selecting and maintaining all supplies, literature, and materials needed for the Hospice Grief Program.

Completes reports, documentation and other assignments in a timely manner.

Represents the Hospice department to patients, families, team members, and to the community in a positive and professional manner.

Qualifications /Training:

Must possess a Bachelor’s degree in Counseling, Social Work, Divinity, or other health related field. Must possess a valid South Carolina driver’s license and have available vehicle at all times. At least one year of experience in a counseling related field. Preferably at least one year of experience in a counseling in Hospice or similar setting. Participation in education opportunities dealing with death and dying. 

 

Licenses/Certifications/Registrations/Education:

Must possess a valid SC drivers license and BLS.
